Information For Parents

Special Provisions for Parent(s) or Guardian(s) of Children:

I, the undersigned, as parent or guardian, give my consent for our child, identified herein, to participate in Upfront Sports activities throughout the year. I understand that participation in any activity is done at our own risk. I will not hold Upfront its officers, agents, employees, or anyone acting on its behalf responsible or liable for injury occurring to our child in the course of such activities or such travel.

I understand that Upfront will complete required accident insurance forms, after which all claims under an insurance policy, or policies, for injuries received while participating in an activity, shall be processed by the camper, his/her parent(s), or guardian(s) through the company handling the camper’s insurance policy and not through Upfront.

In consideration of enrolling a child in Upfront Sports programs, the undersigned parent(s) or legal guardian(s) hereby grant explicit consent to Upfront Sports and its affiliates for the capture, use, and distribution of images or videos featuring the enrolled child for any commercial purpose. This includes but is not limited to promotional materials, marketing campaigns, and online platforms. The undersigned affirms that they have the legal authority to provide such consent and acknowledge that Upfront Sports retains the right to utilize the aforementioned media content perpetually. This consent is voluntarily given without coercion and the undersigned release Upfront Sports from any liability arising from the use of such images or videos in accordance with the terms stipulated herein.
